Why do I need to complete a pre‑visit form?
Your visit involves more than just signing on and speaking with Dr. Hoover. We are required to follow state and federal regulations, especially for medication management. The pre‑visit form helps us keep a complete and accurate history of your treatment, verify required information, and safely prepare your care.
Does this policy apply to telehealth and in‑office visits?
Yes. This policy applies to ALL visits—both telehealth and in‑office.
For every appointment, we ask that the pre‑visit form and payment be completed ahead of time, preferably at least 24 hours before your appointment, so your visit can proceed without delays.
Why does completing the form early matter?
Completing your form in advance allows us to review your information, check required databases, prepare medications, and address concerns before your visit. Same‑day forms increase wait times and may delay prescriptions.
Why is payment required at least 24 hours before my appointment?
Payment at least 24 hours before your appointment allows us to confirm our schedule, prepare for your visit, and offer canceled appointment slots to other patients who need care. We are a small clinic, and this process helps us stay organized and better serve you and our other patients.
When can I complete the pre‑visit form and payment?
Forms and payments may be completed up to one week before your appointment and must be completed at least 24 hours before your visit to qualify for discounted pricing.
Will my visit fee increase if I complete everything on time?
No. Your visit fee will NOT increase if both the pre‑visit form and payment are completed at least 24 hours before your appointment.
What are the visit fees?
Monthly MAT visits are $225
Monthly visits are $200 when the pre‑visit form and payment are completed at least 24 hours in advance
Quarterly MAT visits are $325, due before the scheduled visit
If quarterly balances are split into payments after the visit, a $25 processing fee applies to each installment
What happens if I complete the form but forget the payment?
Both the form and payment must be completed at least 24 hours in advance. If either is missing, the visit is charged the full visit fee.
What if I pay but do not complete the form?
Payment alone does not meet requirements. If the pre‑visit form is incomplete or not submitted, your visit may be delayed, rescheduled, or charged the full visit fee.
What if I complete everything less than 24 hours before my visit?
Same‑day or late completion is allowed, but the visit is charged the full visit fee and does not qualify for discounted pricing. Same‑day completion may also result in longer wait times or delays in prescriptions.
What information must be included on the pre‑visit form?
You must list:
Any other providers or prescriptions since your last visit (verified through the Mississippi Prescription Monitoring Program)
Any refill needs, including non‑buprenorphine medications
Medications not listed in advance may be delayed.
Can I request medication changes or refills if I didn’t list them?
Requests not listed on the pre‑visit form may be delayed or addressed at a later visit to ensure safe prescribing.
What are the rules for telehealth visits specifically?
Telehealth visits require online payment at least 24 hours before the appointment. Payments are processed securely through PayPal. A PayPal account is not required to use a debit or credit card. Cash is not accepted for telehealth visits.
When does my telehealth visit start?
After payment and required forms are completed, your provider will start the telehealth visit when available. Typical wait time is up to 15 minutes.
Can I arrive or log in without completing the form and payment?
No. Visits—whether in‑office or telehealth—do not begin until both the pre‑visit form and payment are completed.
What if I am late to my appointment?
Late arrival or late log‑in may result in a shortened visit, rescheduling, or delays in prescriptions.
How do I complete my form and payment?
